How to Use Online Scheduling Tool
- Choose whether you are requesting a new patient visit or follow-up visit.
- Select the day and time.
- Enter your contact information and a detailed reason for the visit. If you do not specify the reason for the visit your request will be denied.
- You will receive an email once the request has been submitted. If you do not receive this, you may have no submitted your request so try again. If you still do not receive the email a second time, send an appointment request to evonatmed@gmail.com
- The appointment request will be approved or denied within 24 hours. If the appointment cannot be approved, Dr. Estrada will reach out to you via email to discuss specifics.
- Once the appointment is approved you will receive an invitation to create a patient portal account through Charm.
- You will also receive an email prompting you to complete the required consent forms and new patient questionnaire. These forms must be signed and completed online at least 24 hours before your visit. Dr. Estrada will be unable to provide care if she does not receive the necessary paperwork.
- Finally, you will also receive an email with a link for your video consultation. You must join the session by clicking the provided link 10 minutes before you appointment. If you are not seeing the emails in your inbox, please check the spam folder.
